Identifying patients who could potentially be in the last year of life can assist with planning future care. Having conversations about future care can assist the practitioner to be aware of what is important to the patient, and what options are acceptable. CQC require advance care planning discussions to have  been offered to patients identified as being in the last year of life, and for these decisions to have been shared in the wider health and social care system.

This workshop will give you the necessary skills to have these conversations and how to integrate them into your current practice.

Places are limited to 30 per session, so book your place NOW by selecting an available date below

Please note that applications will only be accepted from one GP per practice or one consultant in each speciality per Trust.  If you cannot attend on the day, you will be expected to send a representative from your practice.

About the venue
Chelmsford City Racecourse is situated in Great Leighs, 5 miles north of Chelmsford.  Car parking at the racecourse is free.  The car park is situated at the Main Entrance off the A131 bypass.
